Complications of Lichen Sclerosus in Males

  • Scarring:
    Common, and may lead to urinary problems or dysfunction.

    • Urethral involvement occurs in about 20% of men, which can result in urethral stricture.

    • Surgical options, such as circumcision, may be required.

  • Squamous Cell Carcinoma (SCC):
    Around 4–5% of men with LS may develop SCC of the penis.

    • It remains unclear whether LS itself causes SCC, or whether human papillomavirus (HPV) co-infection plays a role.

    • Long-term follow-up is important. Patients should be advised about warning signs of malignancy, and any suspicious lesions should be biopsied.

  • Dysesthesia and Dysfunction:
    Penile dysesthesia (neuropathic pain) may occur and does not respond to steroids.

    • Topical local anesthetics may help.

    • Dysfunction is possible and can significantly affect quality of life; referral should be offered when needed.

Prognosis in Males

  • Men have a higher chance of remission or cure compared to women.

  • Most achieve remission with ultra-potent topical steroids (50–60%) or circumcision (>75%).

  • In young boys, steroid treatment may prevent the need for circumcision.

  • Scarring cannot be reversed with medication.

  • Recurrence is possible, and in some cases LS overlaps with lichen planus, making management more complex.

Diagnosing Lichen Sclerosus

The diagnosis of lichen sclerosus is most often clinical. Lesions should be carefully documented for ongoing monitoring, ideally using diagrams or clinical photography.

Additional investigations may include:

Biopsy

  • Indicated only when the diagnosis is unclear or malignancy is suspected.

Management of Male Anogenital Lichen Sclerosus 

  • First-line treatment is an ultra-potent topical steroid, applied once daily until remission is achieved, followed by a gradual reduction in use.

  • Relapses may require repeat courses, or intermittent use (e.g., once weekly) to maintain remission.

  • If phimosis persists without improvement after 1–3 months of steroid therapy, referral for circumcision is considered.
